Procedure for Requesting Transcript of Records, True Copy of Grades, Certifications, Certificate of Transfer Credential and Honorable Dismissal

  1. The requesting party secures application form from the Learning Center or at the Office of the University Registrar (OUR) UP OPEN UNIVERSITY, Los Baños, Laguna.
  2. Requesting party files the application at the Learning Center or at the OUR.
  3. OUR processes application and sends the requesting party charges due. OUR prepares the requested documents.
  4. Requesting party submits proof of payment (original copy of the bank validated deposit slip or UPOU-OR).
  5. OUR releases the document.

Releasing of documents:

    • TOR: 1 st copy - one month after receipt of proof of payment
    • 2 nd copy - one week after receipt of proof of payment
    • Certification - one week after receipt of proof of payment
    • TCG - one week after receipt of proof of payment

Notes:

  1. Processing Fee
    1. PHP 30.00/page for TOR and PHP 20.00 for Certifications
    2. US $ 20.00 (if to be mailed abroad)
  2. The documents being requested such as transcripts, certificate of transfer credential and honorable dismissal will be released only to students with duly approved university clearance.
  3. All payments are to be transacted at any PNB branch or at the UPOU Cash Office
